Important Information about Preventive Medicine

Each year it is important to receive an annual visit from one of our Providers

Medicare Patients

If you have Medicare we will recommend an "Annual Well Visit."  This is covered by Medicare annually at no cost to you.  

The first part of your Annual Well Visit will be conducted by one of our nurses - you will then see one of our providers to address any ongoing chronic conditions you might have.

During this visit we might discuss, as needed:

Tobacco cessation

Advanced Care Planning

Depression Screening

Cognition

Diet & Exercise

We will make recommendations to you on services you should receive in the next 5-10 years to ensure optimal health

Commercial Patients

If you have Commercial Insurance we will recommend an annual "Physical."  Again, this is no cost to you and covered under the Affordable Care Act.

During your physical we will address any changes that might have occurred since your last visit and review existing conditions that you have had since establishing care with Washington County Internal Medicine.

We will be happy to fill out your commercial bio metric form at this visit  and only at this visit - this is a requirement from most payers.  If you have had your physical done by another provider within the last 12-months we will ask you to have that provider complete your bio metric form.  If you haven't had a physical in the last 12-months we can schedule one with our providers and fill out your form then.
